Two chambers, two pressures, two purposes. Knowing the difference is the first step to using oxygen well.
Hyperbaric oxygen therapy rests on a principle from basic physics. At everyday atmospheric pressure, almost all the oxygen your blood carries is bound to haemoglobin, and that system is already close to full. Raise the pressure of the air around you and have you breathe oxygen-enriched air, and something changes: oxygen begins to dissolve directly into the plasma, the fluid of the blood, reaching tissues that red cells struggle to supply. The chamber is simply the vessel that makes this possible — but not all chambers are the same.
The two worlds of HBOT
Soft (mild) chambers are flexible, often portable enclosures that pressurise to roughly 1.3–1.5 times normal atmospheric pressure (1.3–1.5 ATA), typically with concentrated rather than pure oxygen. They are gentler, more accessible, and well suited to recovery, rest and general wellness use. Hard chambers are rigid steel or acrylic vessels that reach higher pressures — commonly 2.0–2.5 ATA — and usually deliver 100% oxygen. The medical and longevity research literature has overwhelmingly been built on these higher-pressure, hard-chamber protocols.
This distinction is not a matter of one being "real" and the other not. It is a matter of dose. Pressure and oxygen concentration are the levers that determine how much oxygen reaches the tissue, and the right setting depends entirely on what you are trying to achieve.
The paradox that makes it work
The most interesting science here is counter-intuitive. Repeated, intermittent exposure to high oxygen — followed by a return to normal air — appears to trick the body into mounting the same regenerative responses it would normally reserve for low oxygen. Researchers call this the hyperoxic–hypoxic paradox: the fluctuation itself, not a steady state, is the signal. It is thought to activate hypoxia-related pathways that drive new blood-vessel formation, mobilise stem cells, and prompt cellular housekeeping.
It is the swing in oxygen — up, then down — that the body reads as a call to repair.
What the strongest studies show
In a 2020 prospective trial at the Sagol Center in Israel, thirty-five healthy adults aged 64 and over completed sixty daily hyperbaric sessions. The researchers reported that the telomeres of several immune cell types — the protective caps on our chromosomes that shorten as we age — lengthened by more than twenty percent, while the number of worn-out "senescent" cells fell. Two hallmarks of biological ageing moved in the direction of youth, in a population with no disease driving the change.
A companion randomised controlled trial in the same year found cognitive gains in healthy older adults, linked to changes in cerebral blood flow. And in a 2022 blinded, randomised trial in middle-aged athletes, a hyperbaric protocol improved markers of mitochondrial respiration and physical performance. The common thread across this work is the protocol: higher-pressure, hard-chamber sessions delivered repeatedly over weeks.
Does milder pressure reach the cell?
This is the question that matters most for everyday wellness use, and the evidence is more encouraging than the pressure gap suggests. The headline telomere findings above were produced at 2.0 ATA — but the mechanism behind them appears to switch on at far gentler settings. In a controlled 2023 study, a single hour at mild pressure (1.4 ATA) and at high pressure (2.5 ATA) produced a strikingly similar rise in the oxidative-stress and antioxidant signalling thought to set the regenerative cascade in motion. The cellular trigger — the hyperoxic–hypoxic signal itself — does not appear to require the highest pressures.
Honesty about that same study cuts the other way too: not everything was equal. The immune and inflammatory responses scaled in proportion to the oxygen dose — more pressure, more response. There is a related point about anatomy, as the brain in particular appears to respond well within a milder window of roughly 1.3–1.75 ATA. The fair conclusion is not that soft and hard are identical, but that milder protocols genuinely engage the same core cellular machinery — which is exactly why they earn a credible place in a recovery- and longevity-minded routine.
The protocol is the intervention
Whichever chamber you choose, the result lives in the schedule, not the single session. The cellular trial that produced those telomere changes did not rely on one dramatic visit — it used sixty sessions, each around ninety minutes, delivered five days a week over roughly three months — and, tellingly, the benefits peaked near the thirtieth session, evidence that the body adapts over a sustained course rather than all at once. Two further details define a proper protocol. Sessions are typically broken by brief air breaks — short returns to ordinary air mid-session — which is precisely the oxygen "swing" the paradox depends on, and which also lowers the risk of oxygen toxicity. And clinical courses generally run from twenty to sixty sessions depending on the goal, at frequencies ranging from several per week to one or more per day. Time, interval and repetition are not the logistics around the treatment. They are the treatment.
So which chamber is for you?
If your interest is the cellular and cognitive effects documented in the trials above, the evidence points to structured, higher-pressure protocols. If your interest is recovery, rest and feeling restored as part of a broader routine, a milder protocol may be entirely appropriate — and far more practical to sustain. The honest answer is that the chamber should be chosen to fit the goal, not the other way around.
A word on safety and standing: in Switzerland and the EU, hyperbaric chambers are regulated medical devices, and there are real contraindications — certain ear, lung and cardiovascular conditions among them. This is precisely why oxygen is best used under qualified supervision, with the protocol matched to the individual rather than bought off a shelf.
